#14th ICanCaRe Tutorial for Certified Tobacco Cessation Specialists

On September 16, 2023, the 14th tutorial for Certified Tobacco Cessation Specialists took place. Dr. Mariya, Dr. Siddhi, and Dr. Anshika effectively coordinated the program. The agenda for the tutorial included:

1. **Demonstration of School Program**: Conducted by Dr. Surbhi Jindal, Nodal Officer NTCP at AMBALA.

2. **Group Discussion on Cases Presented**:

   – Dr. Megha Srivastava discussed a case of a young individual consuming 30 cigarettes per day, aiming for a cold turkey approach in their quit journey.

   – Mrs. Pravesh shared a case about a 15-year-old boy who has started smoking, and the discussion focused on how to approach and intervene.

   – Dr. Mariya shared a success story from the previous tutorial where her patient successfully quit and even referred new patients. Dr. Pawan suggested creating a testimonial video for the ICanCaRe YouTube channel.

   – Information was shared about the availability of tobacco-related medications for the clinics at special prices for CTCS.

   – Dr. Pawan Gupta emphasized with CTCS acknowledging ICanCaRe in all presentations, whether online or offline.

ICanCaRe’s Participation at the 6th IAPSM NEZCON at Assam Medical College, Dibrugarh

The ICanCaRe Academy was prominently represented by Dr. Pawan Gupta at the 6th Indian Association of Public and Social Medicine North East Zone Annual Conference held at Assam Medical College, Dibrugarh.

ADVISE QUIT TOBACCO CAMPAIGN

Dr. Pawan Gupta, M.Ch. in Surgical Oncology and Founder Director of ICanCaRe, along with his role as Senior Director of Surgical Oncology at MAX Institute of Cancer Care, is a MASTER trainer for Tobacco Cessation Intervention in India. He’s a pioneer in promoting Tobacco Cessation intervention within the medical community and beyond. Dr. Gupta’s #advisequittoabcco campaign urges healthcare professionals to tackle the root cause of various diseases by motivating patients to quit tobacco through the “ASK, ADVISE, and REFER” approach to a Tobacco Cessation specialist. Dr. Gupta emphasized that no disease can be fully cured if the patient continues to consume tobacco. It is both a moral responsibility and ethical practice for all healthcare professionals to incorporate Tobacco Cessation.

NEWER STRATEGIES TO IMPROVE QUIT RATE TO 100%

During his talk on “NEWER STRATEGIES FOR TOBACCO CESSATION,” Dr. Gupta presented how the #ABCDtobaccocessation approach has achieved a 100% quit rate. The ICanCaRe Tobacco Cessation protocol, which includes specific medicines and a holistic approach with active intensive follow-up, has proven successful in helping patients quit tobacco. Dr. Gupta also demonstrated TrisCaRe the innovative mouth-opening device for betel nut users.

CAPACITY BUILDING FOR TOBACCO CESSATION INTERVENTION

The #ICanCaReAcademy (www.icancare.in/academy) is dedicated to capacity building for Tobacco Cessation Intervention and has called upon IAPSM to collaborate in creating a significant impact on society. The #SAVEtheyouth initiative encompasses 360 interventions, targeting all aspects of society for effective tobacco control in alignment with the Sustainable Development Goals.

Recognizing tobacco as a complex disease and the root cause of numerous health issues, ICanCaRe’s capacity-building program aims to reach across India through the #SAVEtheyouth campaign. The SAVE the Youth program involves sensitizing against tobacco, assisting users to quit through a digital protocol intervention, building a network of Tobacco Marshals, Tobacco Ambassadors, and Tobacco Cessation Specialists, and enforcing a tobacco-free society.

Dr. Gupta’s extensive research has led to the establishment of a simplified digital Tobacco Cessation protocol, and he is also the author of several books on this subject. Through the ICanCaRe Academy, over 1,000 doctors have been trained on successful ICanCaRe protocols for effective tobacco quit programs.

ICanCaRe extends its gratitude to the organizing team of the 6th IAPSM NEZCON, especially Dr. Suneela Garg and Dr. Tulika Goswami, for the invitation to this enlightening conference.

This event provided a valuable opportunity to meet and discuss collaboration with stalwarts of the PSM community, including Dr. Ajit Kumar De, Dr. Dabadeep Kalita, Dr. Rashmi Ahmed, Dr. Sthapana Sharma, Dr. KMS Mech, Dr. Ship Shekhar Datta, Dr. Manoj Balaji Patki, Dr. Subhajit Bora, and Dr. Subrata Baidya from various Medical Colleges of the North East. They expressed enthusiasm for the ICanCaRe protocol and its potential to significantly impact.

COLLABORATION WITH IAPSM

Dr. Purushottam Giri, National Secretary of IAPSM, acknowledged the promising beginning and stressed the importance of propagating and refining the developed interventions. He encouraged the adoption and dissemination of proven methods to combat the challenging addiction to tobacco.

Several young doctors at the conference expressed interest in learning and implementing the ICanCaRe protocol in their future practice, highlighting the promising future of the #ICanCaRe initiative for a tobacco-free society.

Cessation using the Cold Turkey Method for Asthmatic patient

CTCS Tutorial #13 Case 2

 

On 15 September 2023, the 13th monthly tutorial session was conducted for Certified Tobacco Cessation Specialists. One of the two cases discussed a 38-year-old, chronic cigarette smoker, owner of a travel agency, referred for cessation by a pulmonologist. He had been smoking for the last 25 years, consuming around 40 cigarettes a day. He was married, had 2 children, and was accompanied by his mother to the cessation center. He gave a history of bronchial asthma for 3 years and diabetes, and blood pressure for 2 years, and was under medications for the same.

            He had been consuming 40 cigarettes in a day, with maximum consumption during morning hours, around 6-7 cigarettes reportedly before using the washroom. Apart from this, he had increased frequency while driving, meeting friends, after meals, and under stress. He stated that a few times when he could not smoke for 2-3 days, he felt extreme anger, stress, and restlessness. He reported sweating, thirst, constipation, and increased appetite during this phase. Presently, he was quite motivated to quit smoking due to health issues.

            He was explained the adverse effects of smoking and the benefits of quitting in the long run. A detailed treatment plan was made for him after counseling. He was guided for a Cold Turkey method of quitting smoking, replacing it using a 21-mg Nicotine patch to be used for three weeks and Nobacco gums (4 mg) to be used for severe craving episodes. He was allowed 2 puffs of smoking a day initially, which he stopped after 2 days. He complained of body cramps and worsening asthma in 2-3 days of withdrawal. He was supplemented with SleepNova, Lycoedge, Cbinan, ELC, and Mg tablets.  He was suggested to use the Acapella device for breathing exercises, which helps in clearing mucous in the throat. Mucolytics and anti-asthmatic drugs were also modified accordingly by the consulting pulmonologist. He was kept on a nebulizer, steam inhalation, and a modified diet plan and showed considerable improvement in asthma over 2 weeks.

The Nicotine replacement patch would be reduced to a 14-mg dose after 3 weeks, further to a 7-mg dose in 2 weeks, continuing, in addition with 1mg BD Varnitrip, over 3 months duration. He would be regularly monitored by a cessation specialist and pulmonologist over this period and the treatment plan would be modified accordingly.
